Personal Training: Getting Started

Please come to the CFC main office (319 Helen Newman Hall, Mon-Fri, 8:00 AM - 4:30 PM, closed at lunch) to register and/or purchase services. After registering/paying, you will be contacted within 5 business days to schedule your appointment. If you have questions, you may contact the program coordinator (Sue Fine) at

  • A "Fitness Intake" is a required first appointment for all services other than body composition testing.
  • Prior to attending the Fitness Intake meeting, you must print and fill out the following forms: Fitness Intake Packet and the PAR-Q (Physical Activity Readiness Questionnaire). 
  • This paperwork must be completed and brought to the fitness intake meeting. The gathered information will be reviewed by one of CFC’s Personal Trainers with you and will not be released to anyone other than program staff without your permission.
  • The Fitness Intake forms will be collected following the intake meeting. The information collected from the forms allows us to learn about your goals, preferences, availability, injuries, health history, etc., and to determine if medical clearance is needed.
  • After attending this meeting, if no medical clearance is necessary, you will be able to purchase services. If medical clearance is required, it will need to be received prior to purchasing services. Procedures for getting medical clearance are discussed at the intake meeting.
  • Payment is required at the time of purchase. One must have a current CFC membership, as well. Please bring your Cornell I.D.
  • Medical clearances are good for one year, unless health status changes.
